New Student Sign Up Form

Hi there!

Please fill out the following short form so that you have access to the scheduling website I use: My Music Staff

• You will receive a link to complete sign-up at your email address. You may login to the portal at: http://app.mymusicstaff.com/

• You can use the portal to: review lesson notes and attachments, reschedule and cancel lessons, and more.

• SMS Lesson Reminders, will be sent 1 day before lessons to your phone number via My Music Staff.

• Monthly Invoices will be sent your email address and are due on the 1st of each month.

• Student Phone Number and E-mail are optional. If provided, My Music Staff will forward SMS lesson reminders and lesson notes to them.

• Please fill out the address field if I am traveling to you for in-home lessons
